Before your visit to Jones & Jones Medical Associates, please review your health-plan booklet or call your benefit office or insurance company

to clarify your specific policy benefits and requirements.

on your insurance card, or at your benefits office. We have included a list of our contracted carriers for your convenience.

 

Our staff will not be able to answer questions regarding specific plan details.

Participation with Jones & Jones Medical Associates is determined by your employer, and your specific insurance carrier/network.

The best way to find out if your Insurance company participates with Dr Richard F. Jones is to call your insurance carrier.

 

We accept most health insurance plans for services performed at Jones & Jones Medical Associates.

You will be asked to provide proof of insurance at the time of service to determine eligibility.

 

Commercial Insurance plans also generally have deductibles, co-pays, and/or coinsurance.

 

A co-pay is a set amount due each visit. The cost is determined by your insurance policy. This usually does not count toward your deductible.
